Partnerships for Wellbeing have been involved with NHS Highland Long Term Conditions Collaborative in the ‘LET’S GET ON WITH IT TOGETHER’ project  since  the first meetings in Fort William.

The Long Term Conditions Collaborative within NHS Highland are committed to supporting people with long term conditions to self manage and to strengthening partnerships with voluntary and other organisations to provide supported self management opportunities.

During 2011-2012 PfW hosted the post of the Co-ordinator for the LGOWIT project.  Funding for this post came to an end in Sept.   The Core Steering Group for the project, of which PfW is a member, will continue to drive the project until such time as further funding can be sought.
